All organisations are going to have some huge screw ups in their time. Most of them hush them up and publicise the things that go well. But for an organisation like MI5 it's best if everyone else thinks you're incompetent, it makes life a lot easier. So they do the opposite, make sure that everyone knows how stupid they can be and keep quiet when things go well.
Which means either they are very smart or incompetent or both at the same time. It's hard to be entirely sure.


But then since this is a government body it's probably just good old fashioned stupidity.
