1. First determine what is actually running, unfortunately Windoze doesnt have a good tool for this. So download the freeware Hijackthis to see what is actually running.

Go line by line through he running applications and google the application name for the ones you don’t recognize, remove the bad ones.

2. Then download Spybot and run through its update cycle, then immunize against bad programs, then run its search for the badware, always make backups and dont delete anything your not sure of :-)

3. You mentioned antivirus - I use a couple to make sure here are some free scan links:
PandaSoftware

Mcafee

Trend Micro

4. And finally I use a free basic firewall from zonealarm to catch any weirdness not known yet in the wild. As it only takes about 20 seconds to get a bad worm on an unpatched XP system.
