The issue is "if you could avoid negative repercussions."

Would you appear in a pornographic movie if you could be sure that your friends and family would never see it?

Let's reword that with a different "sin" (not my take on it, but the position I feel the questionaire takes):

Would you rob a bank if you could be sure that no-one would know it was you who did it?

It's not worded to ask would you tell anyone, it's worded to ask would you do this bad thing if no-one (in this case no-one likely to condemn you) would know?
