... at least up to 5 years ago, which was the last time I held a licence, car registration includes a requirement for insurance for medical costs of the other party. Anyone with a brain will also insure against damage to the other partie's vehicle (called 'bomb insurance', as it is very useful if your 'old bomb' isn't worth insuring, but you don't want to pay for the porche you hit with it.)
