It means both. It just depends on the context it's used in. For instance, if you were to whisper sweet nothings into your girlfriends ear, hopefully it would be the sexual arousal meaning. However, if you were to walk in on an animal rights convention wearing a fur coat, then it take the angry meaning. I hope that clears it up for you.
