but I didn't see any comments about OS and drivers and the like. Are you confident that all that is setup properly ?

It is very easy to make mistakes that degrade the signal quality of even the most expensive tp cable. That includes stretching, sharp bends and untwisted ends but also wiring faults such as split pairs.
I have seen cable pairs wired as 1&2, 3&4, 5&6, 7&8 (even in custom cables from a computer shop!). The correct pairing is 1&2, 3&6, 4&5, 7&8. Of course cable mistakes become more significant the longer the cable is.
You could try forcing the NIC to 10Base-T which is a little more forgiving to bad cables.
