To me, this suggests that there is an environmental factor, if it is not caused by the virus infecting specific cells as suggested - if A happens, then HIV infects the cells, else it lies dormant. Perhaps that would explain the supposed drug connection - the drugs catalyze A allowing HIV to infect the cells, causing some people to think that the drugs were the cause not HIV. And, of course, our own immune system could be A as you suggested too.
