Not getting on a merry-go-round any time soon either, it was nauseating enough before your comment.
But do these people really need voices? Isn't it more like they lack the inner voice of reason, logic and a conscious(ness)? Neither of your examples are all that extreme, if you look at it with a naive mind: Dump in lettuce? Whaddaya think cavemen used as toilet paper? Getting Johnson out on a merry-go-round? Not like clothing is a natural concept etched into our DNA.
Now, as for voices, they generally seem to help people not only go beyond normal, socialized lines of control but also beyond natural psychological limits, forgoing empathy etc.
As for my personal voices, they are either critiquing me or stopping me from doing something: "Nah-ah-ah! Turning car into oncoming traffic is counter-productive!"
Urges to plan heavy, blunt objects into heads of people come all by themselves :-)
