I don't have good answers to all of these, that is why I asked for ideas. I will answer what I can.
I would allow the candidate to take the test whenever they felt ready, but those who fail could not retest for at least a year from the previous attempt.
It would be a test of mental maturity, responsibility, and the understanding that actions have consequences.
Grading criteria would depend on the test itself.
As the test should be strictly a test of maturity, there should be no need for exceptions.
Most rites of passage, if they are not entirely symbolic, only test a specific skill, not overall fitness for society.
