I'm running PHP 4.0.4pl1 (I really need to sort that out!) and all appears OK. I inserted the following two lines into common.inc between lines 25 and 26.

printf("<!-- \$matches[0] = %s -->\n",$matches[0]);
printf("<!-- \$matches[1] = %s -->\n",$matches[1]);

This shows up in the source of the output file as:

<!-- matches[0] = /usr/local/httpd/htdocs/phpix/index.php -->
<!-- matches[1] = /usr/local/httpd/htdocs/phpix/ -->

We appear to be trying to perform a chdir to the same directory as the script we're currently executing (seems a little pointless to me but there's got to be a reason). Best I can suggest is if this is failing, then you've either got a permissions problem (e.g. apache user has no execute perm in htdocs), an apache config problem (e.g. DocumentRoot incorrectly set), or a PHP bug. I'd check the first two myself...
