IIRC (and I could be wrong), there was just Hades, and that's where everyone went. I'm not sure how the Elysian Fields fit in.

If I remember my Egyptology, when you died your soul was balanced against a feather on a set of scales. If the scales balance, you move on to the happyness of the netherworld. If your soul is heavier than the feather (because you've been a bad, bad person), you get fed to the Eater of Souls.
