1) That is always possible, but if you really want to see it you can change your settings to allow you to view moderated posts. (the "My Settings link on the left, then check the box marked "Show comments that have been moderated" and then click the "Save user settings" button. You'll be good to go. 2) That's a good question. I'd say they have been going up for a while now. I'd say it's because of more moderators, a lot of people that feel the need to test the rules, people deliberately posting things they know they should not, and people making mistakes.
