They are:

Baptisim
First communion (Holy Eucarist)
Confirmation
(these first three are the rites of passage and the only ones required to be a Catholic)
Holy Orders (Becoming a Priest, Sister, or Brother)
Marriage
Confession (this is more or less required, but not going doesn't make you not a Catholic. On the other hand, you have to go before you can do certain things - like get married or have your First Communion)
And, finally,
Sacrement of the Sick
