should really only apply to the name a person was given in a Christian ceremony at their christening, their confirmation, etc..

Given name covers any name other than your surname (family name), or a patronymic.

First name is just the first of your given names.

Middle name is really rather arbitrary. It can be all of your given names except the first, it can be any one of your given names other than the first, or it can be your patronymic if you have one.

A patronymic is a name denoting descent from your father (like Johnson meaning son of John). These are not commonly used in the U.S.. I don't know if they are common elsewhere, but I think they are still used sometimes in Russia.

Last name is your surname which is your family name.
