I've checked and can find none of merit. Perhaps your resources are better in this case. My observations (sounds kinda pretentious, don't it?) were that the term was not used at all in the regions I lived in until after his Ice Cream man routine. I think it was in Delirious or Raw, but not much is available to me for verification. So, if your sources of information or observations can shed any more light on this matter, pray tell.
