As I see it, you need to ask yourself two questions:

1) What are your current health costs? Subtract this from what you would pay in premiums. This represents your normal net loss on having the insurance.

2) What is your comfortable risk level? Are you prepared to deal with the cost of a medical emergency without this insurance?

Compare the answers to these two questions. Does the cost of answer 1 outweigh the risk of answer 2?

Without knowing the plan, it's hard to say what the company is paying. I have paid for my own health insurance before, and I would say that with this plan your company is paying more than half of the premiums. (This includes the "bonus" they offer for you not to take insurance.)
